Please provide your name, street address, email address, and phone number to Cleveland Mini Vacations Sweepstakes, by completing the online entry form. Entries must be transmitted by 11:59 PM E.S.T. on August 4, 2002. By participating, each entrant accepts these rules. 2. Limit one entry per household. 3. Sweepstakes sponsors are not responsible for lost, late or misdirected entries. No telephone calls, mail or facsimiles will be accepted regarding this sweepstakes. 4. Entries will not be acknowledged or returned. 5. All entries become property of the Convention & Visitors Bureau of Greater Cleveland. 6. This sweepstakes is open to everyone 18 years old and older living in the United States except the employees of the Convention & Visitors Bureau of Greater Cleveland, the sponsors, their advertising agencies, suppliers and members of their immediate families. 7. Monthly winners will be chosen June 3, 2002, July 1, 2002 and August 12, 2002. Winners will be notified by postal mail and/or email. The odds of winning depend on the number of eligible entries received. 8. Cleveland Mini Vacations Sweepstakes packages valid June 3, 2002 through October 20, 2002 only. Check participating venues for exact days and hours of operation. The stay is based on availability at the participating hotel. There may be blackout dated during holidays. Other restrictions may apply. 9. Only four prizes will be awarded. Unclaimed prizes will not be awarded. 10. No substitutions in prizes or cash will be allowed. Sponsors reserve the right to substitute the prize with one of greater or equal value. 11. The winner and any persons accompanying the winner must comply with all instructions in the prize notification letter, complete an eligibility affidavit and release of liability, and return them to the Convention & Visitors Bureau of Greater Cleveland within ten days of the date on the prize notification letter; otherwise an alternate winner will be selected. The winner must also agree to the use of his or her name and/or photograph for advertising and/or trade purposes where permitted by law. 12. Liability for any applicable federal, state or other taxes will be the sole responsibility of the individual winner. 13. This contest is subject to all federal, state and local laws and regulations. 14. For the name of the winners, log on to www.travelcleveland.com/minivacations periodically.
